Before we move forward with your project, we ask for a brief health declaration. It serves only the safety of the tattoo — to identify contraindications — and lets us and the artist decide responsibly whether and how the project proceeds.
1. What it is and isn't
- It is a RAVEN safety screening, completed online, before booking.
- It is not a medical assessment or diagnosis, and does not replace consulting a health professional.
- It is not the informed consent for the act of tattooing (risks and aftercare) — that is given by the artist, at the session, in a separate document.
2. How we handle your health data
Health data is a special category (Art. 9 GDPR). We process it on the basis of your explicit consent (Art. 9(2)(a)), with:
- Minimisation: only safety questions, in Yes/No — we do not collect medical history, diagnoses or medical documents.
- Restricted access: only those who need it (RAVEN, in the triage, and the executing artist, if the project proceeds).
- Short retention: kept only until the triage decision (plus a safety margin); then deleted or anonymised. Not part of any long-term archive, not mixed with other data.

3. The declaration (Yes / No)
Answer truthfully. When in doubt, answer Yes — we'll confirm with you.

Do not provide diagnoses, medical documents or third-party data. If you answer Yes to anything, we may confirm what is needed for safety — without asking for your medical history.
4. Confirmation on the day of the session
If the project proceeds, the artist confirms with you, on the day, any changes: the state of the skin at the time, fever/infection/wounds, recent medication, alcohol/substance use in the previous 24h and — for long projects — pregnancy. This is not collected online; it is an in-person safety check.
5. What you declare by submitting
- Your answers are true and complete, to the best of your knowledge.
- You will communicate any relevant change before the session.
- You understand the triage is not a medical assessment and that, when in doubt, you should consult a health professional.
- You understand that false statements or omissions may compromise the safety and liability of RAVEN and the artist.
- You understand that RAVEN or the artist may decline, postpone or condition the project for safety reasons.
6. Explicit consent (Art. 9) — checkbox text
A dedicated, separate box, not pre-ticked, distinct from accepting the Terms and from the contact consent. Gating: the form does not advance without it.
"I give my explicit consent for RAVEN (and the executing artist, where applicable) to process the health data I provide, solely to assess the safety of the tattoo, under the Privacy Policy. I understand I can withdraw this consent at any time, without affecting processing already carried out, and that without it RAVEN may not be able to proceed safely with the project."
If you do not consent (supporting text): "If you don't give consent or don't provide information essential to safety, RAVEN or the artist may be unable to assess the project and may postpone or decline it for safety reasons."
7. Safety decision
Where there is a sign of a contraindication, we may ask for clarification, suggest a medical statement, postpone or decline — always for safety, never as a judgement of the person. If symptoms appear after the session (infection, reaction, poor healing), we direct you to a health professional.
